It asks questions about how you have been feeling over the past two weeks. Respond quickly and intuitively.

This self-assessment includes 21 short sets of statements. For each one, read through all the answer options first. Then choose the one that best describes how you’ve been feeling over the past two weeks—including today.
If more than one answer feels true, go with the one that feels most true—or choose the one with the highest number. That helps reflect your experience more accurately.
